do they have to cut a small patch of the fabric from a bit of spare seam and blend it in ?
or do they pick threads that match and build it from scratch ?

Both. The latter method is far more difficult and expensive. Luckily, the back of the trousers has loads of extra fabric that will be cut out when I have the trousers taken in. The reweavers can use this fabric.
